At Comfort Keepers, we provide uplifting in-home care for seniors and older adults who require a helping hand, a supportive companion, and day-to-day assistance in their home. As a Homecare Services Coordinator you will have the opportunity to support our caregivers in delivering one-on-one care that enhances quality of life of our clients and their families.*Absent a legally recognized exception, this position requires proof of your fully vaccinated and boosted status for COVID-19.Job Summary:The Homecare Services Coordinator, is responsible for customer service and relationship management ensuring that clients receive the highest quality of care and are extremely satisfied. Is responsible for orchestrating, monitoring and supervising the care and communicating care coordination details to all team members, other providers, and family members. Is also responsible for covering all requests for service, and resolving scheduling crisis. Directly manages a team of caregivers and reports to the office General Manager. Schedules shifts and hours 2 weeks or more in advance, by coordinating caregiver qualifications and availability to client's needs.Identifies and addresses scheduling issues (i.e. call offs, no shows, etc.) and uses independent judgment to evaluate alternative solutions and implement established policies for immediate resolution.Uses independent discretion to manage work/schedule priority based on client/business needs and engages the proper resources to develop an effective and efficient schedule. Analyzes, evaluates and forecasts current hours and staffing levels against established sales goals and makes recommendations to aid in maintaining proper staffing levels. Creates schedules that maximize the client experience, meet caregiver needs, and control overall agency costs (e.g., overtime)The Homecare Services Coordinator, you will be responsible in part or in whole for the active recruitment, hiring, training and retention of the caregiving staff.Responsible for working with General Manager to ensure that all compliance requirements for the caregiver staff are met and maintained, including any state specific regulations that must be followed, as well as company implemented policies.Works highly collaboratively with the Client Care Coordinator, HR, and Sales teams to ensure the platinum experience for clients and highly motivated, engaged caregivers.May be required to work on-call as business needs requireQualifications:High School diploma and two years related experience and/or training; or equivalent combination of education and experience. College degree in related field preferred. Knowledge of office procedures and health care also preferred.Requires proficiency in word processing and computer skills (Office, Excel). Will need to master additional software including ClearCare.Must possess above average human relations, customer service, problem-solving, and organizational skills. Must be able to work under time pressures and manage multiple demands simultaneously. Extreme attention to details is essential. Excellent telephone and communication skills are necessary. Must be a team player willing to share information and work cooperatively with other members of the management team and caregiving staff.Essential Functions:.Proactively manages the ongoing internal client relationship for existing clients. Uses independent judgment to accomplish problem resolution, increasing support for clients, and quality assurance. Anticipates needs and advises families.Selects and apportions work among caregivers based on skills, performance and overtime requirementsIdentifies and makes recommendations regarding any safety concerns of caregivers or clientsDirectly responsible for regular and routine performance evaluations of all managed staff of caregivers. Assists in regular and routine assessments of current clients, including identifying and making recommendations regarding any safety concerns of caregivers or clients.Follows up with caregivers after each new assignment. Able to make termination recommendations to HR.Determines client recovery activities for de-activations, hospitalizations, hour's reductions, and cancelled hours. Responsible for ensuring that clients value our services as outstanding.Coordinates care with Client Services team, Client Care Coordinator, family members, and other care providers. Reviews daily care logs to ensure quality care is delivered. Initiates care coordination process based on information gathered in the notes with the necessary parties' including the External ICC.s, and documents accordinglyFollows all office communication and documentation protocols to ensure excellent service to clients, staff, potential and inactive clients.Participates in the Manager on-call rotation by carrying the agency's emergency cellular phone on a rotating basis. While on-call, handles all client, caregiver, and scheduling issues that occur and conducts all new client intakes as needed.Participates in formal and impromptu management meetings and in the development of systems to improve care coordination, client satisfaction, employee performance and retention and office operations.Assists the other members of the management team as needed. Completes special projects as assigned.Answers potential client inquiries whether made via phone or web and sells and qualifies them for home visits.Manages new client assimilation process and ensures it is completed in a specified time frameThis job description is not intended to be all-inclusive. Position Physical Demands:Walking, sitting, and standing with lifting limited to files and records typically not expected to exceed 15 pounds in weight.
