Canterbury Woods, Pacific Grove, CA.This is a full-time exempt position with full benefits.The Director of Social Services and Activities is an integral part of the team responsible for the planning, developing, organizing, implementing, evaluating and directing the social service functions and activities in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to ensure that the psychosocial needs of the residents are met. Assists residents and their families to understand and cope with resident’s personal, emotional and related health and environmental problems. Also ensuring that the spiritual development, emotional, recreational, and social needs of the resident are met/maintained on an individual basis. As life care residents move back and forth along the continuum from independent to assisted living and skilled nursing living, the Director of Social Services and Activities supports and assists residents and their families with these transitions and case management capacity.ResponsibilitiesPlans, develops, organizes, implements, evaluates and directs activity/wellness programs to ensure all residents assessed needs are met.Develops and maintains a good working rapport with other departments within the facility, and outside community health, welfare and social agencies to ensure that the emotional needs of residents are assessed, monitored and reassessed to ensure quality of care and life. Participates as a member of the Interdisciplinary Team.Supports and assists residents and their families with life transitions, i.E. location transfers, terminal disease diagnosis, chronic disease diagnosis, loss of spouse, etc. at all levels of care.Ensures quality care and resident rights are respected at all times.Serves as an advocate for the resident and is notified and follows up with any issues related to the psycho-social well-being and mood state of each resident, especially when there is an absence of family support.Ensures a written assessment of each resident is completed within seven days of admission.Ensures a Social Services plan is developed, including goals and treatments for each resident who needs such services, with input from the resident, the family, the resident’s care physician and the interdisciplinary team.Ensures sections AA, AB, AC, A, E, F, Q1A, 1B, and 1C of the MDS is completed promptly as well as the Resident Assessment Protocol (RAP’s) on a timely basis and adds those to the care plan.Ensures documentation of written progress reports in the Social Services section of the resident’s health record on quarterlies, changes of condition, or “as needed” progress reports.Participates in all care plan meetings and any meetings as necessary. Communicates with Interdisciplinary Team to determine date for care conference, then, calls family to schedule a time.Assists in arranging transportation, adaptive equipment and clothing necessities.Makes recommendations and implements the discharge plans for all the residents who require a temporary or permanent move from their independent, assisted living, or skilled nursing accommodations as well as short term rehabilitation and non life care residents working within the interdisciplinary team. Supports and assists residents and their families with these transitions in both a counseling and case management capacity. Also resolves conflicts which may occur between families and the resident, resident and the facility staff, families and the facility staff and between resident and resident.Maintains consumer information boards appropriately and ensures resident rights are posted and reviewed regularly.Participates in Residents’ Council monthly meetings when invited.Makes recommendations on changes in policies and procedures to Health Services, as well as assisting in the development, administration, and coordination of department policies and procedures.Keeps updated on current Federal and State regulations, as well as professional standards.Performs all documentation and charting duties accurately and on a timely basis.Maintains confidentiality of all pertinent patient/resident care information to ensure patient/resident rights are protected.Assists in the development of and provides regularly scheduled orientation and in-service training programs in relation to the social and emotional needs of the residents/patients, residents rights, resident abuse reporting.Reviews Skilled Nursing and Assisted Living resident complaints and grievances and makes necessary oral/written reports to the Administrator.Participates in Facility Emergency Plan training and fire drills, and if an emergency or disaster occurs, immediately assumes the pre-assigned emergency duties. Also reports all incidents/accidents, and all unsafe/hazardous conditions/equipment immediately.Works well under pressure; meeting multiple and sometimes competing deadlines. Will, at all times, demonstrate cooperative behavior with staff, colleagues and supervisors.Performs other duties as deemed necessary and appropriate, or as may be directed by the supervisor. Participates in the overall quality assessment and improvement programs.Knowledge, Skills, and AbilitiesMust be able to read, comprehend, write and speak English proficientlyMust have firsthand knowledge of interior design, precise interpretation of floor plans, move coordination and downsizing issuesAbility to coordinate multiple details and provide appropriate, accurate informationAbility to work with residents, family members, and advisorsAbility to communicate detailed information verbally and in writingKnowledge of Microsoft Office, demonstrates technical literacy, and operates a personal computerUnderstanding of and commitment to the Community Vision Statement and the core values of mutual respect, collaboration, service, orientation, and diversityAbility to assess issues and make recommendations for solutions or improvementAbility to organize, schedule and prioritize multiple projects and priorities to meet constantly changing deadlinesAbility and willingness to learn software as neededBe flexible and willing to maintain a high standard of excellence and commitment to success of team and interdepartmental relationships.Excellent telephone skills.Education and QualificationsTwo years experience in a social or recreational program within the past five years, one year of which was full-time in a patient activities program in a health care settingCollege degree requiredGerontology credential preferred3-5 years experience in residential care or skilled nursing.Ability to read and speak well and be understood in the English language.Must have attention to detail, follow through and good interpersonal/problem solving ability.Satisfactory completion of physical examination including TB test.Fingerprint clearance from Department of Justice.Good understanding of long term care, Medicare, Title 22, and OBRA regulations.
