Roles & Responsibilities Coordinate patient care and transitions
across the hospital and post-acute settings to achieve safe, timely
discharges Perform utilization review using InterQual criteria to
ensure appropriate level of care Develop and document
individualized plans of care in Epic, keeping charts accurate and
timely Facilitate family meetings and collaborate with patients,
families, and community partners to support discharge goals Arrange
DME, SNF placement, and ambulance transport independently and
efficiently Work closely with interdisciplinary teams including
social services and transportation for comprehensive care
coordination Apply knowledge of DRG and CMS regulations to support
regulatory compliance and appropriate billing classification
Respond to weekend rotations and department needs while maintaining
high-quality case management Qualifications Active California
license required (pending license accepted but must be licensed by
start date) Minimum 2 years of acute care hospital case management
experience with demonstrated care coordination and discharge
planning Experience with utilization review/InterQual criteria and
computerized documentation (Epic required) Travel/contract
experience preferred; first-time travelers can be screened and
considered Comfort with weekend rotations (every other weekend) and
department staffing needs Proficiency with Microsoft Office and
electronic charting systems Knowledge or experience with DRG and
CMS processes and expectations Education Not specified Knowledge &
Skills InterQual criteria and utilization review processes Epic EMR
and computerized documentation best practices Care coordination,
discharge planning and plan of care development Arranging DME, SNF
placement and ambulance transport Familiarity with DRG, CMS and
regulatory requirements Working with patients experiencing
homelessness, substance use treatment options, and mental health
crises Family meeting facilitation and interdisciplinary
collaboration Experience with HMOs/IPAs and community resources
