About the Position Job Title: Case Manager Profession: Registered

Hours per Shift: 8 Experience: Minimum of 2 years of hospital
experience in Acute Care Case Management or Discharge Planning is
required. License: California state license is required. Pending
licenses accepted but must be licensed upon start date.
Certifications: Must have relevant certifications. Must-Have:
Experience in assessing patients, care coordination, discharge
planning, and utilization review is essential. Ability to organize
and participate in family meetings. Knowledge of treatment options
for drug/alcohol abuse, advance directives, and protective services
processes. Competency in arranging durable medical equipment,
skilled nursing facilities, and ambulance transport independently
is required. Description: The position entails managing a caseload
of up to 25 patients. The nurse will coordinate patient care,
including care planning and discharge activities. The role demands
participation in the development of treatment plans. This position
involves working collaboratively in an interdisciplinary team,
including social services and physical therapy. Travel is required
between facilities. Weekend shifts are necessary to meet
departmental needs, specifically every other weekend. Providers are
expected to float between assigned facilities and refusal to float
may lead to termination of the assignment. The nurse must have the
ability to perform tasks independently and effectively manage the
