Registered Nurse (RN) | Home Health Location: La Quinta, CA Agency: Cynet Health Pay: $2,607 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in La Quinta, California, 92253! Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of two (2) years nursing experience in a clinical setting with a frail or elderly population preferred. License: Unencumbered Registered Nurse (RN) License required. Certifications: None specified. Must-Have: - Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations. - Ability to initiate venous access for IV hydration or to draw labs. - Experienced with administering IM, SC, or transdermal injections and providing wound care. - Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations. - Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members. - Ability to work independently with minimal supervision. - Demonstrated ability to prioritize in a fast-paced environment. - Experience and competency working with people from diverse backgrounds and cultures. - Commitment to unlocking the full potential of our most vulnerable seniors. Description: Conduct home health nursing assessments to determine the nursing, personal care, and equipment needs in the home. Regularly re-evaluate participant nursing needs. Initiate the plan of care and make necessary revisions as participant status and needs change. Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions. Include the participant and their family in the planning process. Deliver and document home health nursing interventions as agreed upon in the participant's care plans. Maintain a healthy and safe environment. Promptly and accurately respond to physician orders. Correctly administer medications and perform ordered tests and treatments. Instruct the participant, family, and caregivers regarding the disease process, self-care techniques, and prevention strategies related to their medical conditions. Remain alert to input from team members, participants, and caregivers. Update the interdisciplinary team promptly of any changes in participant condition or medical status. Manage smooth care transitions between settings including hospitals and home. Identify discharge planning needs as part of the care plan development. Implement discharge planning prior to participant discharge. Instruct, supervise, and evaluate home health aide care provided every two weeks. Participate in end-of-life care coordination and support. Provide backup coverage to other nursing functions as needed. Complete all required documentation in a timely and accurate manner. Protect privacy and maintain confidentiality of all procedures and information regarding participants and families. Follow policies and procedures and participate in required quality improvement activities, staff training, and meetings. Communicate regularly with the supervisor and team regarding workload and priorities. Complete all mandated trainings and education on time. Exercise flexibility in performing assignments as business needs evolve.
