The role requires strong clinical judgment, excellent communication skills, and the ability to work independently while coordinating with a multidisciplinary team.Key Responsibilities:
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Administer medications, treatments, and wound care as prescribed.
  • Educate patients and families on disease management and home care techniques.
  • Maintain accurate documentation using EPIC/Oasis charting systems.
  • Travel to patient homes within the designated service area.
  • Ensure compliance with all healthcare regulations and standards.Requirements:
    • Certification: BCLS (Basic Cardiac Life Support) required.
    • Experience: Minimum 1 year of skilled home health nursing experience.
    • Transportation: Must have reliable personal transportation.
    • Weekend Commitment: One weekend shift per contract.
    • Oasis Documentation: Required for patient assessments.Additional Information:
      • Scrubs: Any color permitted.

      • Mileage reimbursement follows IRS standards.
