Intake Coordinator (Bilingual Preferred)/Case Manager. Can you make

HOSPICE INTAKE COORDINATORAs a Bilingual Hospice Intake
Coordinator, you serve as the liaison between patients and staff.
You conduct the intake process by gathering complete data of
prospective clients, processing intake data from referral sources,
and educating those making referrals regarding reimbursement,
insurance coverage, and appropriate utilization of services. When
needs have been evaluated, you identify and verify insurance
coverage as well as obtain approval for the initiation of service.
Friendly and personable, you explain financial information to the
client, including insurance benefits and any costs for which the
patient will be responsible. You also take the initiative to obtain
a funding source or supply source for patients without adequate
income.In addition to working with clients, you complete
administrative tasks around the office, such as coordinating
visits, maintaining patient schedules, and verifying authorization
for treatment. Other administrative duties include recording
accurate and specific directions to each patient's home and
notifying team members of any address changes. As you collaborate
with staff and program partners to help clients, you build positive
working relationships. You are always professional, respectful, and
conscientious about confidentiality. You get great satisfaction out
